Complete State Hub bootstrap workplans (WP-0001)

- Review integration files; fill SCOPE where templated
- Document dev workflow in stack-and-commands.md
- Seed WP-0002 implementation workplan; mark bootstrap finished
- Hub sync via fix-consistency
