482edcd7eb chore: register with Custodian State Hub 
Add CLAUDE.md (session protocol, tool boundary, workplan prefix BRIDGE-WP)
and workplans/ directory. Repo registered as ops-bridge under custodian
domain (id: 1bf99f56).
